Cardio Flow Design Co., Ltd. establishes the Terms of Use for the ‘ACHD App for ACHD Patients (tentative)’ Personal Health Record (PHR) application as follows:

By using this service, it is assumed that you agree to all the terms and conditions in this agreement.

Article 1 (Purpose):
These terms and conditions are established for the use of the ‘ACHD App for ACHD Patients (tentative)’ Personal Health Record (PHR) application (hereinafter referred to as ‘the Service’) operated and managed by Cardio Flow Design Co., Ltd. These terms and conditions specify matters to be observed and applicable matters in using the Service.

Article 2 (Definitions):
For the purposes of these terms and conditions, the following terms have the meanings set forth below:
‘Cardio Flow Design Co., Ltd.’ refers to a company founded by two active physicians who believe that the study of hemodynamics will greatly impact cardiovascular medicine. The URL of the company’s website is https://cfd.life/.
‘ACHD App Member’ refers to an individual residing in Japan who has registered as a member by following the procedures established by Cardio Flow Design Co., Ltd. and whose registration has been approved by Cardio Flow Design Co., Ltd.
‘Registration Information’ refers to all information provided by Service users to Cardio Flow Design Co., Ltd. for the purpose of using the Service.
‘Personal Information’ refers to information within the Registration Information, including name, phone number, date of birth, gender, email address, test results, and other information that can identify or specify an individual. It may also include information that can identify a specific individual through cross-referencing with other information.

Article 3 (Scope of Application):
These terms and conditions apply to all relationships between Cardio Flow Design Co., Ltd. and Service users concerning the Service.

Article 4 (Notification and Contact to Service Users):
When Cardio Flow Design Co., Ltd. needs to notify or contact Service users, it will do so through appropriate methods, including email, LINE or other electronic messages, Short Message Service (SMS), phone, postal mail, based on the registration information of the Service user.
In cases where Cardio Flow Design Co., Ltd. needs to notify or contact all Service users, it may choose to make announcements within the Service in lieu of the methods mentioned in the previous paragraph.

Article 5 (Member Registration):
Registration as an ACHD App Member is done after agreeing to these terms and following the prescribed procedures in the Service.
Registration as an ACHD App Member is considered complete when approved by Cardio Flow Design Co., Ltd.
Individuals eligible for ACHD App Member registration must be residents of Japan.
Cardio Flow Design Co., Ltd. may decline membership registration or revoke membership for the following reasons:
(1) Falsification of required information during registration.
(2) Failure of email or messages to reach the member due to changes in the member’s email address, and other similar issues.
(3) Violation of these terms and conditions.
(4) Other reasons deemed inappropriate by Cardio Flow Design Co., Ltd.

Article 6 (Change of Registration Information):
Service users must promptly notify Cardio Flow Design Co., Ltd. of any changes to their registration information using the prescribed method within the Service.
Cardio Flow Design Co., Ltd. will not be responsible for any disadvantages incurred by Service users due to failure to notify changes as required in the previous paragraph.

Article 7 (Disclaimer):
Except in cases of intentional or gross negligence, Cardio Flow Design Co., Ltd. shall not be held responsible for damages or disadvantages incurred by users or third parties resulting from the use or inability to use the Service.

Article 8 (Changes in Service Content and Suspension or Termination of Service Delivery):
Cardio Flow Design Co., Ltd. may change the content of the Service and suspend or terminate delivery of the Service due to system maintenance, inspection and updates, force majeure events such as natural disasters and power outages, and other reasons. Cardio Flow Design Co., Ltd. will make efforts to notify users in advance, but in cases of emergency and unavoidable circumstances, this notification is not required. Cardio Flow Design Co., Ltd. shall not be held responsible for any damages or losses incurred by users or third parties due to changes in the content of the Service or suspension or termination of service delivery.

Article 9 (Changes to the Terms of Use):
Cardio Flow Design Co., Ltd. may make changes to these terms and conditions as deemed necessary. In such cases, Cardio Flow Design Co., Ltd. will notify Service users, make announcements within the Service, or use other appropriate methods to inform users in advance. The revised terms of use will take effect from the date specified in the notification or publication.
